Home or hub

We bring it to you, or you come in

Most clients use both: home while recovery is new, a hub when the gym helps. Your therapist stays the same.

Three hubs

Hubs in Liberty, Independence and Raymore

Each hub has treatment rooms and a gym. Therapists start at a hub and drive out from there.

Where we go

Six counties, within about 30 miles of a hub

We visit homes, apartments and senior living communities in the towns below. Call to ask about a town that isn't listed.

  • Clay County: Liberty, Kearney, Smithville, Excelsior Springs, Gladstone, Pleasant Valley · Within about 25 miles of the Liberty hub
  • Platte County: Parkville, Platte City, Riverside, Weston · Within about 25 miles of the Liberty hub
  • Jackson County: Independence, Blue Springs, Grain Valley, Oak Grove, Raytown, Grandview, Lone Jack · Within about 25 miles of the Independence hub
  • Cass County: Raymore, Belton, Harrisonville, Peculiar, Pleasant Hill, Archie · Within about 25 miles of the Raymore hub
  • Ray County: Richmond, Orrick, Lawson · Within about 30 miles of the Liberty hub
  • Lafayette County: Odessa, Higginsville · Within about 30 miles of the Independence hub

We don't visit homes more than about 30 miles from a hub. If you live farther out, call us: you may be able to come to a hub or join a visit by video.

Getting started

From your call to your first visit

  • You call or send a message

Give us your name and number, and tell us if you would like care at home or at a hub. Please keep health details for the visit.

  • We check your benefits

We look at what your plan pays for a home visit and for a hub visit, and call you before we book.

  • You choose home or hub

Pick what suits the week. You can switch next time.

  • You meet your therapist

The first visit takes about an hour. You and your therapist agree on goals and a plan.

Referring providers

Send a referral to the nearest hub

Physicians, case managers and discharge planners can send a client to the hub nearest them or ask for a home visit. One line reaches all three hubs.

How it works

  • One number and one form for all three hubs

  • A therapist calls your client within one working day

  • We send your office a note after the first visit

Before you call

Do I need a referral?

In Missouri you can start physical therapy without a referral, within limits your therapist explains. Your plan may still ask for one, and Medicare asks a doctor or another provider to sign your plan of care. We tell you what applies before the first visit.

Can I switch between home and a hub?

Yes. Many clients start at home and move to a hub when they can travel, and some do the reverse after a setback. Tell the front desk and we will move your next visit.

Which towns do you visit?

Homes within about 30 miles of a hub in Clay, Platte, Jackson, Cass, Ray and Lafayette counties. Call us about a town that isn't listed.

Do you take Medicare?

We bill Medicare Part B and many other plan types. Medicare Part B can cover outpatient physical therapy at home if you are not getting home health at the same time. We check your coverage and tell you before we book.

What do I need to have ready?

At home, a chair with arms, a clear stretch of floor and comfortable clothes. At a hub, comfortable clothes and shoes you can exercise in.
